Dear Rautela Ji,
The process is as follows (which I have followed in making my domisile)

1. Your details should be in Parivar Register of village as well as Bahi Khata

2. There are total 4 forms
    a) Parival Register which village sarpanch will seal & sign
    b) Self details form which Patwari & then Kaanango will verify & sign
    c) Request form for Domisile with personal deails
    d) Domisile certificate with diclaration

The process is as follows
3. Your village Sarpanch will check & sign the Pariwar Register,
    Fist Patwari then Kaanango of your refective block will check & verify the details
    In your distt court have to submit a diclaration/ reason of non residence of Uttarakhand on 10/- stamp paper
    Submit all the documnets to the court/ DM office & it will process the documents

Thanking you so much for my mail  rapply. Mam My name is alrady submit in pariwar Register/ bahi Khata of my village tehsil. Kiran ji, my home town is Bhagwali(Ranikhet) but i lived in Delhi .Mam kya mushe village aakhar hi process start karna hoga ya  if  any other option on the process.
